No crowds or rushing into stores to buy gifts. All you have to do is fill out the form and the gift cards will be ordered for you. Participants pay face value for the gift certificates and Fusion receives a rebate from participating stores (see the percentages listed on the “One Stop Shop” Order Form below). 100% of that amount will be deposited directly into your child’s Fusion team account. You can sell them to your friends & family or even do your daily/weekly shopping using these certificates and YOUR FUSION PLAYER benefits! NOTE ABOUT SHIPPING FEE: we are charged a flat fee of $7.75 to ship each order to us. If your order is the only one in the club order that week, then the $7.75 shipping fee will be your expense. However, when your order is part of a larger order, we will prorate your portion of the shipping based on the # of cards in your order to the total # of cards in that shippment. Any amount of the $7.75 shipping fee that is not your expense will be deposited into your team account along with your rebates. You will be notified what that amount is at that time.
Payments by cash or check made payable to "Fusion FC" must accompany orders and given to Lisa Moore, One Stop Shop Coordinator, on Mondays at NTCP Field 4, from 6-7:30pm. Orders/payments can also be mailed to:
